Causes of endometriosis

Stages of development of ovarian endometriosis

Ovarian endometriosis is often referred to as “missed” pathology. And no wonder, because the disease in most cases develops very slowly.

Etiopathogenesis

To date, there are many hypotheses about the causes and mechanisms of development of endometriosis: implantational (transfer of menstrual blood with viable endometrial cells into the fallopian tubes and further into the abdominal cavity), genetic, hereditary, hypothesis of immune disorders and others.

At the same time, despite the huge number of studies conducted by leading international experts, many aspects of endometriosis remain unexplained. Most often, several causal factors and developmental mechanisms are involved in the occurrence and development of the disease. The main role is assigned to retrograde throwing of endometrial cells and such molecular-biological processes as cell proliferation and invasion, the formation of new blood vessels (angioneogenesis), a decrease in the body's regulation of cell death processes (apoptosis).

A large number of immunomodulators, proinflammatory cytokines, growth factors and sex hormones are involved in the development of foci of endometriosis and their progression. This is also due to a defect in the endometrial stroma cells, which produce a local excess of progesterone on the background of resistance to it, as well as an excessive amount of prostaglandins, which, together with cytokines, act as mediators of inflammation, pain and infertility.

All these mechanisms are largely determined by genetic disorders or changes, are interconnected and function in such a way that apoptosis, angioneogenesis, the immune response and the chronic inflammatory response contribute to the proliferation of endometriotic tissues.

Hormonal imbalances, intrauterine medical manipulations, the lack of timely implementation by the woman of her reproductive function and environmental pollution by industrial and transport waste (benzophenols, dioxins), which increase the risk of endometriosis by 2 times, also have a certain value.

Disease classification

  1. I st. - the presence on the surface of the ovaries, on the peritoneum, lining the rectal uterine space of point endometrial foci without cavities characteristic of cystic formation.
  2. II st. - localization of a characteristic cyst with a diameter of not more than 5-6 cm in one of the ovaries, as well as the presence of small endometrioid inclusions on the pelvic peritoneum.
  3. III Art. - the same signs as in stage II, but in combination with unexpressed adhesions in the uterine appendages (tubes and ovaries) without participation in the adhesions of the intestines.

Endometrioid ovarian cysts in this disease are also called chocolate.

They are formed at the site of the follicle during the period of ovulation (how to determine the day of ovulation, you can read in this article). The walls of the latter are lined with functioning, endometriosis like glandular cells, trapped in the ovaries by retrograde (one of the versions) and which are hormonally dependent. Therefore, they function in accordance with the menstrual cycle (menstruate). As a result, blood containing iron (in hemoglobin) appears in the cavity of the cyst, which is oxidized. Blood ages, acquires a thick or glassy texture and chocolate or tarry color.

An endometriotic lesion on the surface of the ovaries has the appearance of merging with the tissue of the latter or of individual foci (nodes, nests) with a dark, thick liquid elongated, round or irregular in shape. These formations are also called "eyes."

Ovarian endometriosis treatment

Laparoscopic treatment of ovarian endometriosis

The goal of treatment is to:

  • removal of foci
  • reducing the intensity of pain
  • fertility therapy
  • preventing the progression of pathology,
  • relapse prevention.

The main directions of treatment are:

  1. Surgical

  3. Combined.

Drug therapy

Consists of the use of hormonal drugs for:

  • treatment of cysts of small sizes (up to 3 cm),
  • when a woman refuses surgical treatment,
  • in the presence of contraindications to surgery.

Medications can be prescribed medroxyprogesterone acetate, Dienogest, combined oral contraceptives, Danazol, Gestrinone, as well as gonadotropin-releasing hormone agonists with subsequent hormone therapy (estrogen in combination with progestin) or Gn-Pr antagonists. The effect of hormone therapy is to inhibit ovarian function. The hormonal Mirena intrauterine device has a similar effect.

Combined treatment

It consists in prescribing hormonal drugs in order to prevent the recurrence of ovarian endometriosis after surgical treatment or when it is doubtful that all small foci are removed during surgery.

In addition to these groups of drugs prescribed symptomatic therapy aimed at:

  • reduction of pain intensity (analgesics, nonsteroidal anti-inflammatory drugs, Paracetamol),
  • normalization of psychosomatic status (sedatives and anticonvulsants, antidepressants),
  • reduction of inflammatory processes and a decrease in the intensity of adhesions formation (physiotherapeutic methods - magnetic therapy, low-frequency impulse currents, hydrotherapy in the form of bischofit and coniferous baths with a water temperature not exceeding 36 o), climatic treatment with the exception of excessive insolation, thermal and physical, including electrophysical procedures.

Currently, a single algorithm and consensus on the treatment of patients with endometriosis is not developed.

Classification of Ovarian Endometriosis

  3. III degree: the development of adhesions, violation of the patency of the fallopian tubes.
  4. IV degree: an increase in the diameter of cysts up to 5 cm or more, the spread of adhesions to the organs of the digestive tract and urinary system.

In patients with endometriosis, characteristic so-called "chocolate" cysts are formed. Their formation is directly related to the period of ovulation. Due to the fact that the ovaries are covered with endometrial tissue, then at a certain point of the menstrual cycle they begin to reject, that is, menstruate. As a result, cysts are filled with blood from the injured vessels. Gradually, the blood coagulates and acquires a dark chocolate shade and a specific tarry consistency.

  • women who forget to change intrauterine devices in a timely manner, which puts their health at risk of developing not only endometriosis, but also cancer,
  • with the abuse of spirits, as well as drugs and tobacco,
  • the presence of inflammatory processes in the genitals, which have a chronic type of course, and sometimes worsen,
  • the presence of autoimmune diseases and chromosomal abnormalities,
  • hormonal dysfunction.

Scientists have found that endometriosis does not occur at all. It is especially common in women who:

The translocation theory, otherwise known as transport, explains the pathology described by the fact that endometrial cells with menstruation enter the peritoneum and other organs, and the center of endometriosis is also fixed there.

  • foci may be surrounded by a thick and dense capsule,
  • foci have a homogeneous structure and high echogenicity,
  • Heterotopes are oval or round.

Treatment methods

Treatment of ovarian endometriosis is aimed primarily at removing foci of the disease, reducing pain, treating infertility, and only then at preventing the progression of the disease and preventing its recurrence.

Treatment may be conservative, surgical, and combined. Let us consider in more detail each of these areas.

Folk therapy

Treatment of endometriosis of the ovaries with folk remedies is carried out in combination with traditional therapy or as a prophylactic measure. Only herbs to get rid of endometrioid cysts is impossible.

To prevent the formation of adhesions, you can use decoctions of the needles of coniferous trees (spruce, pine, cedar, etc.).
